For parents and carers who would like a childcare place at Meadows Tree Family Hub.

What happens next: we will acknowledge your enquiry within five working days, invite you to an open day before we open, and contact you with session availability and fees as soon as places are released.

Please note: completing this form registers your interest only. It places no obligation on you and does not guarantee a place. Places are allocated on the date your form is received, with priority given to siblings of children already registered.

We are open Monday to Friday, 6.00am to 7.00pm, 51 weeks a year (closed between Christmas and New Year). Please tick the sessions you would ideally like for Child 1. If your pattern differs for Child 2, or varies week to week, please explain in Section 6.

Meadows Tree Family Hub (Meadows Tree Ltd) is the data controller for the information on this form. We use it to respond to your enquiry, understand likely demand, manage our waiting list and discuss suitable childcare options with you. Our lawful basis is legitimate interests, and consent where you tick the marketing option below.

We will keep enquiry details for up to 24 months from your last contact with us, or until you ask us to delete them, whichever is sooner. We do not sell or share your details with third parties for marketing. You have the right to access, correct or delete your information, or to withdraw consent at any time, by emailing info@meadowstree.co.uk. If you are unhappy with how we handle your data you can complain to the Information Commissioner’s Office (ico.org.uk).

Your form is sent securely to our admissions team at info@meadowstree.co.uk. You can also email a copy to info@meadowstree.co.uk or hand the form in at an open day.
